Before submitting all documents, please print your name and Chico I.D. on the top of each requested item. All documents received will be scanned and linked to the student's file. Paper documents are retained for three years and then shredded. Please submit only copies and keep originals for your records. If you choose to submit documents by fax, please call our office immediately at 530-898-6451 to confirm receipt and legibility of the fax transmission.
